My Medical History is tailored for use by a patient. Here the patient prepares his/her own medical history a well as the One Page Medical Summary which is automatically developed as the medical history is prepared. This summary can be used to assist in your care if an emergency arises. Patients can carry their summary in their wallet or purse or they can keep a copy in their athletic or golf bags.

·        Preparing your medical history is template driven with key information requested with appropriate prompts along the way.

·        Because it is template driven, the need for typing is limited; hence frustration, valuable time and effort can be saved.

·        All of the individual components of the history can be viewed, saved and\or printed using BML's unique prose outline format. Our format is usually similar to the one used by physicians in their offices.

The final result is an individual medical history for 2010 that is complete, accurate, savable, printable and secure. Our software is not dependent on the internet, a website or a server used by the hospital or physician. Only you have the ability to decide for whom and under what circumstances your information will be available.

The physician’s medical history software, “Patient Medical History 2012” offers a template system for obtaining the patient’s complete medical history; saving it as an electronic medical record file of the office, emergency department or hospital; printing it in total or in part. In addition the physician can provide the patient with a One Page Medical Summary of the key components in his/her medical history.

There are also versions for residents to use in training, for medical students to use during their clinical years of training as well as versions designed to meet the needs of Allied Health Care Providers in the office or during their education.

The versions for Medical Students, Allied Health Students, Residents and Allied Health Providers were revised in 2009, there are no 2010 versions. The 2009 versions can be ordered at a 10% discount off the standard price of $29.95.
